SUBSTANCE: invention relates to an anesthetic aid for thoracic operations in elderly and senile cancer patients with concomitant cardiovascular pathology, the said invention includes catheterization of the epidural space before surgery and administration of a test dose of 2% lidocaine 2.0 ml. Catheterization of the epidural space is carried out at the level of the Th5-Th6 vertebrae, induction of anesthesia is performed with the use of 0.05 mg/kg of midazolam, 0.1 mg/kg of propofol, 0.25 μg/kg of fentanyl, and 0.4 mg/kg of ketamine. Rocuronium bromide is administered at a dosage of 0.6 mg/kg. Intubation is performed using a double-lumen endobronchial tube, a central venous catheter is installed in the subclavian vein and anesthesia is maintained using the inhalation anesthetic sevoflurane with a MAC of 0.7–0.9. Analgesia is performed by intravenous fractional injection of fentanyl at an average rate of 0.03 mcg/kg/h, as well as 0.375% ropivacaine at a rate of 2–3 ml/h into the epidural space. Lidocaine is administered as a bolus at a dosage of 0.75 mg/kg over 20 minutes, then placed on a continuous infusion at a dosage of 1 mg/kg/h.

EFFECT: method reduces the risk of developing cardiovascular complications in the early postoperative period due to the administration of lidocaine.
