FIELD: medicine; pediatric surgery; gastroenterology.

SUBSTANCE: intended for the complex treatment of dynamic intestinal obstruction (DIO) in children with acute pancreatitis (AP). The method includes the administration on the 1st day after hospitalization of a hyperosmolar solution with a temperature of 15-20 degrees, the rate of administration and volume of which depend on the age of the patient. The residue is monitored by active aspiration from a nasointestinal probe. Prolonged analgesia is performed through an epidural catheter. On the 2nd day, the volume of the administered hyperosmolar solution is increased, the rate of administration and volume depend on the patient age. When DCN resolves, nutritional support is provided with a hydrolyzed mixture on the 3-4th day; the rate of administration and volume depend on the patient’s age, for up to 2 days. On the 4-5th day, an isocaloric polymer mixture is administered; the rate of administration and volume also depend on the age of the patient. When administering an isocaloric mixture, a drug based on pancreatin 5000 IU is prescribed for each administration of the mixture. If the dynamics are positive, on the 6-7th day the probes are removed and oral fractional nutrition is prescribed with a polymer hypercaloric mixture using the sipping method, taking a drug based on pancreatin 5000 IU for each feeding.

EFFECT: method is simple, safe, and makes it possible to increase the effectiveness of treatment of dynamic intestinal obstruction in children with acute pancreatitis due to an integrated, optimal approach to all pathogenetic links of the disease, taking into account the characteristics of different age categories of pediatric patients.
