FIELD: medicine; oncology; radiation therapy.
SUBSTANCE: invention can be used for stereotactic radiation therapy of localized kidney cancer in inoperable patients. Three radiopaque markers are installed sequentially into the kidney tumor while holding the breath with a distance between adjacent markers of at least two centimeters to visualize the markers in the frontal, sagittal and axial planes. If the tumor size is small, a marker is installed next to the tumor, but not further than two centimeters from it. An individual fixing mattress is made. The patient is placed on a fixing mattress in a supine position, arms above his head, and air is pumped into the mattress until the patient is firmly fixed in it. 7 days after the installation of radiopaque markers, cone beam computed tomography (CBCT) is performed on free breathing using an individual fixation mattress with the patient in the position on his back, arms above his head, with a vest previously put on the patient with infrared markers indicating the position of the chest in space. Planning is carried out at an 80% isodose using 200 or more non-coplanar irradiation fields with an indentation of 3 mm into the planned irradiated volume. Immediately before the treatment session, body position is monitored by obtaining an x-ray image of the spine and comparing the position of the spine relative to its position on CBCT. Then the position of the markers in the kidney is monitored. If the markers are displaced by more than 5 mm from their location at the time of CBCT, a new radiation therapy plan is created. Treatment is carried out on a linear accelerator with a breathing control system, delivering a single focal dose of 15 Gy to a total focal dose of 45 Gy in 3 fractions in free breathing using an individual fixing mattress in the supine position, arms above the head.
EFFECT: method provides effective treatment without the risk of increased toxicity due to the precision delivery of high single and total focal doses.
