FIELD: medicine.
SUBSTANCE: invention relates to oncology and radiotherapy, and can be used for staged radiation therapy in treating metastatic brain tumors in breast cancer. Staged radiosurgery is performed with a single focal dose (SFD) of 10 Gy with an interval between sessions of 2 weeks to a total focal dose (TFD) of 30 Gy for 3 sessions. Magnetic resonance imaging (MRI) of the brain with intravenous contrast enhancement is performed. Three-layer stereotactic mask is prepared for the patient before the first session. Topometric spiral X-ray computed tomography (CT) of the brain is performed before each session. Further, in the radiation therapy planning system using these diagnostic CT, MRI, clinical volumes are determined: GTV is formed along the edge of the visible tumor, CTV and PTV according to standards of radiosurgical treatment and critical structures, therapeutic doses are prescribed. Radiation therapy is carried out on a linear accelerator with 6 MeV photons with a single focal dose of 10 Gy per 1 fraction. Repeated contrast-enhanced brain MRI is performed on 14th day from the first stage of the staged radiation therapy with determination of new clinical volumes: GTV, CTV, PTV. Further, the planning system generates a radiation therapy plan for the second stage of radiation in accordance with the prescribed doses. Contrast-enhanced brain MRI is performed on 28th day of the second stage of the staged RS. That is followed by the third stage of radiation therapy with determination of new clinical volumes: GTV, CTV, PTV. Radiation therapy is combined with dexamethasone 4.0 mg/ml twice a day, intramuscularly.
EFFECT: method provides higher effectiveness and tolerance of the staged radiation therapy due to the use of ultra-precise fixing devices, dynamic control of the radiation treatment and its correction.
