SUBSTANCE: device for primary fixation of unstable pelvic fractures in polytrauma is an external fixation device (EFD) and consists of Shants rods and parts of the Ilizarov apparatus, an arch with holes, two telescopic rods and threaded rods, made in the following two sizes: 115 mm and 70 mm, dies, brackets with threaded shank, bolts, nuts and washers. S-shaped curved washers are put on the threaded rod, pressed by the concave side of the S-shaped bend to two telescopic rods by counter-screwing the nuts on a 70-mm threaded rod connecting the two telescopic rods in the middle parts. The device contains two arc segments with holes; 115-mm threaded rods are made double with fixation along the periphery to the dies. In the middle, 115 mm threaded rods are fixed on arc segments with holes. The external frame of the AVF is assembled and adapted to the size and configuration of the pelvis, providing a degree of freedom in three planes and the possibility of stabilization and reposition of pelvic bone fragments by moving the fixation units of individual parts of the external frame of the AVF and the Shants rods.

EFFECT: optimal use of the device for unstable pelvic fractures in the acute period of polytrauma, adaptation of a pre-assembled device to any size and configuration of the pelvis with a maximum degree of freedom of movement of the fixation units of individual parts of the external frame of the device and the Shants rods in any of three planes, the ability to achieve not only immobilization of unstable pelvic fractures, but also reposition of bone fragments, elimination of the lack of rigidity of the external frame of the device in the form of deformations, distortions and play of both the repositioning units and the entire AVF system, the ability to avoid using bone clamps, retractors, an orthopedic table and other devices when using the device.
