METHOD OF DETERMINING INDICATIONS FOR UPPER CERVICAL SYMPATHECTOMY FOR CAROTID THROMBOSIS Russian patent published in 2024 - IPC A61B8/06

Abstract RU 2811292 C1

FIELD: medicine; neurosurgery.

SUBSTANCE: invention can be used to determine indications for upper cervical sympathectomy for carotid thrombosis. Transcranial Doppler sonography is performed to determine the linear velocity of blood flow in both middle cerebral arteries. Additionally, transcranial bilateral monitoring of linear blood flow velocity in both middle cerebral arteries is carried out for 6 minutes with assessment of the amplitude of B-waves of linear blood flow velocity in the range of low-frequency oscillations from 8 to 50 MHz using spectral analysis of time series. The dynamics of focal neurological symptoms are assessed before and after blockade of the ipsilateral sympathetic ganglion. A decrease in the initially increased amplitude of B-waves of linear blood flow velocity in the ipsilateral middle cerebral artery by more than 2.5 cm/s and regression of focal neurological symptoms are considered an indication for upper cervical sympathectomy. In the absence of focal neurological symptoms, the indication for superior cervical sympathectomy is determined on the basis of a decrease in the initially increased amplitude of B-waves of linear blood flow velocity in the ipsilateral middle cerebral artery, which was initially increased by more than 2.5 cm/s.

EFFECT: method makes it possible to non-invasively determine the indications for upper cervical sympathectomy for carotid thrombosis due to the ability, under physiological conditions, to assess the state of the neurogenic component of cerebral autoregulation in this pathology after blockade of the ipsilateral ganglion.
