FIELD: medicine; maxillofacial surgery.

SUBSTANCE: invention is intended for the treatment of edentulous fractures of the upper jaw. In case of a fracture of the edentulous upper jaw of the Le Fort II or Le Fort III type, a 2.0 cm incision is made in the area of the zygomaticofrontal suture. Then a through hole is drilled in the zygomatic process of the frontal bone with a drill above the fracture gap. Through the formed hole, the first end of the steel ligature is inserted towards the orbit and removed from the side of the orbital surface of the zygomatic process of the frontal bone. Next, the first end of the steel ligature is bent around the zygomatic process of the frontal bone. The first and second ends of the steel ligature are grasped in the clamp and passed into the vestibule of the oral cavity, sliding along the inner surface of the zygomatic bone. An incision is made on the mucous membrane of the oral vestibule along the transitional fold in the projection of the first molar and the ends of the steel ligature are removed from the wound canal. The skin wound in the area of the zygomatic process of the frontal bone is sutured. Next, from the vestibular surface at the level of the first and second molars at a distance of 0.5 cm through the mucous membrane in the alveolar process of the upper jaw, two through holes are drilled towards the inner surface of the alveolar process, through which the first and second ones are introduced from the vestibule of the oral cavity onto the inner surface of the alveolar process ends of the steel ligature respectively. The steel ligature is fixed using an oval titanium plate measuring 1.5*0.8 cm in diameter with two holes at a distance of 0.5 cm from each other. The first and second ends of the steel ligature are threaded through the holes in the plate, the titanium mini-plate is pressed to the inner surface of the alveolar process when twisting the first and second ends of the steel ligature together. After 4 weeks, the structure is removed under general anesthesia.

EFFECT: method allows to prevent secondary displacement of fragments, ensures quick and minimally invasive removal of the structure fixing the upper jaw, provides the ability to reposition and fixate fragments of the upper jaw with complete edentia in one stage, is applicable for fresh fractures of the upper jaw according to Le Fort II and III, allows to perform fixation of the upper jaw under conditions of reduced bone density or with finely splintered fractures of the walls of the maxillary sinus due to the combination of techniques of the claimed invention.
