FIELD: medicine.

SUBSTANCE: pre-auricular incision extending to the coronary one is made along the facial median line. The incision is lengthened along the lateral nasal wall, goes about a wing of the nose towards the philtrum and proceeds as a vertical full one of the upper lip. A dissection is performed in a subgaleal layer of temporal and frontal regions up to the border of the maxillary arch. An orbital periosteum is incised in the periocular area along a superior-lateral border. An incision edging with the eye fissure is made. The orbital muscle of the eye is dissected. A lateral border is dissected away from the external orbital wall. The maxillary arch is transected in its posterior one-third. The external orbital wall is transected up to the piriform aperture. A mucosubmucous flap is cut out on the hard palate, thereby exposing the bone. The hard palate is incised along the median line. The maxillary tuberosity is dissected away from the pterygoid process of the sphenoid bone. A facial skull tissue graft is elevated keeping the muscles attached to the bony skeleton. The sinuses and the lateral wall of the base of the skull are exposed.

EFFECT: method provides a wide approach to the base of the skull and the paranasal sinuses, extended manipulations of the pathologically involved tissues.
