FIELD: medicine; surgical endoscopy.
SUBSTANCE: invention is intended for treating Lithiasis of Wirsung's duct by means of contact intraductal lithotripsy with thulium laser. Method comprises treating contact intraductal thulium laser lithotripsy using laser fiber with diameter of 160–600 mcm, consisting of two stages, aimed at destruction of shell and nucleus of concretion using two modes of exposure with change of settings of frequency, power and energy of pulse. Lithotripsy is performed with FiberLase U2 apparatus, concrement shell is exposed to “Popcorning” mode with pulse energy of 0.10–0.20 J, frequency of 150–400 Hz for 1–60 minutes, power of 25–40 W until destruction of the shell into fragments from 1 to 4 mm in diameter. After that, the calculus nucleus is exposed in the “Fragmentation” mode with pulse energy parameters of 4.00–5.50 J, frequency of 8.3–15 Hz for 1–50 min. to the destruction of the calculus nucleus into fragments from 1 to 4 mm in diameter.
EFFECT: method enables to completely destroy large concrements and reduce the exposure time, wherein the treatment is atraumatic for the pancreatic duct system even with a short-term exposure to the active beam.
