SUBSTANCE: invention refers to medicine, namely to ophthalmology. Surgical treatment of macular ruptures with preservation of internal limiting membrane is carried out. Trocars are inserted, three-port vitrectomy is performed, platelet-rich blood plasma (PRP) is applied in the macular rupture area with subsequent formation of a fibrin film. Thereafter, PFOC is applied on the fibrin film and then PFOC is removed. With vitrectomy, the vitreous body is removed only in the posterior and middle layers to the equatorial zone, while preserving the vitreous body on the periphery, in the anterior portion and an internal limiting membrane, after which the salt solution is replaced with air. After single drying of retina in projection of optic nerve head, PRP is applied directly in area of macular rupture. Further, the fibrin film is formed, the residues of the liquid part of the PRP in the optic disc projection are removed. Then liquid PFOC is applied on fibrin film, then PFOC is removed by vitreotome with cutting frequency 10,000 per minute, using mode of "instant cuts", in which separate operation of vacuum and cuts takes place. In a particular case, if peripheral vitreoretinal degeneration is detected after vitrectomy, limiting laser coagulation of the retina is additionally performed. In a particular case, the PRP is applied using 27G cannula with 38G silicone tip in amount of 0.05 ml.

EFFECT: method allows to reduce operational and postoperative complications, due to complete preservation of internal limiting membrane (ILM) and closure of rupture edges with improvement of visual functions.
