SUBSTANCE: invention relates to the field of medicine, namely to ophthalmology. For surgical treatment of regmatogenic retinal detachment complicated by retinal tears, vitrectomy is performed with the separation of the posterior hyaloid membrane, drainage of the subretinal fluid, step-by-step introduction of a perfluoroorganic compound (PFOC) into the vitreal cavity, removal of the pre-stained internal boundary membrane (IBM) by maculorhexis and final tamponade of the vitreal cavity with gas. The PFOC is injected to the lower edge of the peripheral rupture, a peripheral vitrectomy is performed using a sclerocompressor, then the PFOC is removed from the vitreous cavity to the level of the first branching of the vascular arcades. The membrane blue dual dye is injected into the vitreal cavity, the PFOC is removed, leaving it over the area of the macular rupture. After aspiration of the dye, the PFOC is re-introduced to the vascular arcades, IBM is removed in the PFOC medium. The irrigation fluid is replaced with an air mixture with aspiration of the subretinal fluid from the peripheral rupture. The PFOC is removed, leaving it over the area of the macular rupture. Platelet-rich plasma is applied to the macular rupture, after which the complete removal of the PFOC together with the residual subretinal fluid is carried out. In the area of the peripheral rupture, restrictive laser coagulation is performed. The operation is completed by introducing pure gas or silicone oil into the vitreal cavity.

EFFECT: method eliminates toxic damage to the retina, reduces retinal trauma, increases visual acuity.
