FIELD: medicine.

SUBSTANCE: invention relates to medicine, specifically to ophthalmology. For the treatment of rhegmatogenous retinal detachment, subtotal vitrectomy is performed with a cutting frequency of 8,000 per minute through the ports installed in 3.5–4 mm from the limb in the flat part of the ciliary body, with separation of posterior hyaloid membrane from retina and filling vitreal cavity with balanced irrigation physiological solution. Perfluororganic compound (PFOC) is introduced to lower edge of existing retinal rupture, subretinal liquid is drained, PFOC is replaced with air. That is followed by endolaser coagulation of retina at 0.15–0.18 mW along the edges of all available peripheral retinal ruptures. Additionally, a puncture is made in the flat part of the ciliary body at 3.5–4 mm from the limb for fitting the chandelier into it. That is followed by gradual filling of the PFOC vitreal cavity to the lower rupture edge through one of the ports with one-stage aspiration by vitreous cutter of the balanced irrigation solution through the other port. After endolasercoagulation of the retina, the operation is completed by plugging the vitreal cavity and suturing.

EFFECT: method reduces the duration of surgical intervention and compression on the optic disc, reduces the risk of partial atrophy of the optic nerve in the postoperative period, as well as reduces the number of manipulations with surgical instruments and increases functional results.
