FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to ophthalmology, and can be used for surgical treatment of endophthalmitis. That is ensured by central vitrectomy. Posterior hyaloid membrane flap (PHM) is formed in a premacular sac by reciprocating a membrane loop. Vitreous cutter is used to form a dome-shaped PHM detachment on irrigation mode by translational motion to the periphery of the retina and to remove it. PFOC is introduced to the border of PHM attachment to the retina periphery. Then, sclerocompression is used to extrude a peripheral vitreous body to form a horizontal shaft into a vitreous cavity. Peripheral vitreous body and an anterior hyaloid membrane are removed with the help of a vitreous cutter in shave mode. PFOC is removed from the optic disc. Tamponade of vitreal cavity with air mixture is performed with complete drainage of residual fluid from optic disk. Thereafter, salt solution is fed into the vitreous cavity under pressure of 16 mm Hg. When the level of the solution reaches the ora serrata line, the pressure is increased to 23 mm Hg. Sequence of actions from tamponade with air to filling the vitreal cavity with saline is repeated three times.

EFFECT: invention provides higher clinical effectiveness in endophthalmitis due to complete separation of the infected vitreous body and preventive irrigation measures aimed at maximum cleaning of vitreal cavity with prevention of damage to retinal structures.
