SUBSTANCE: invention refers to medicine, namely to thoracic surgery, oncology. Dissection of cellular tissue with lymph nodes in axillary, subclavian and subscapular regions with preservation of lateral thoracic arteries and veins, thoracic-abdominal vein, intercostal brachial nerve, anterior branches of intercostal nerves. Under binocular optical magnification, lymphatic vessels are separated in a projection of the removed fat. Sequential double coagulation of selected lymphatic vessels is performed with the help of electric generator Force Triad LigaSure or bipolar electrocoagulation with low output power of not more than 30 W with observance of angle between lines of coagulation suture more than 90°.

EFFECT: method enables to detect all possible sources of lymphorrhea and prevent their coagulation, preserve nerve endings, blood vessels of small caliber, not included in the dissection volume, which enables to reduce the manifestations of postmastectomy syndrome in patients after axillary-subclavian-subscapular lymphodissection; does not require formation of musculocutaneous tunnels in the area of armpit, which reduces injuries and duration of surgical intervention.
