METHOD OF RECTAL MUCOSECTOMY IN TREATING FAMILIAL ADENOMATOUS COLON POLYPOSIS Russian patent published in 2024 - IPC A61B17/94 A61B17/00 A61B17/11 

FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to colorectal surgery, oncology. First stage involves endoscopic resection of the rectal mucosa. Hydraulic pad is created at the level of the dentate line, and a linear incision of the mucous membrane is made to a submucosal layer up to 20 mm long. An endoscope with a cap is delivered into the submucosal space; a circular dissection is performed in the submucosal layer for 18–20 cm from the anal canal. Dissected mucous membrane is removed. Danish stent is placed in the surgical area for 7–10 days. Second stage in 1–1.5 months in the absence of malignant growth in the removed preparation is performed by colectomy. Distal edge of an ileum and a rectal wall are skeletonised over length of 1–1.5 cm. Separate serous-muscular sutures are used to fix the serous-muscular layer of the ileum with the serous-muscular layer of the rectum. Intubator is installed in rectum within 5 cm from anal canal.

EFFECT: method provides reducing the risk of malignancy in the rectal wall, improved motor and evacuation function of the remaining rectum, reduced risk of postoperative complications in the form of desmoid tumours of the small pelvis, leakage of anastomoses, rectal strictures, purulent-septic processes in the small pelvis; simplifies the formation of the anastomosis, reduces the risk of sexual dysfunction in men and reproductive function in women, risk of anal sphincter insufficiency and frequent urge to defecate, risk of perianal complications.
