FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to anaesthesiology and intensive care, and can be used for prolonged block of radial nerve, median nerve and ulnar nerve from axillary approach. Feature of this method is that the catheter delivery to the radial nerve, median nerve and ulnar nerve is performed using two Tuohy guide needles, wherein the distal end of the first Tuohy guide needle is successively brought under the radial nerve, and the distal end of the second Tuohy guide needle is brought to a point between the median nerve and the ulnar nerve. Catheters are delivered at distance of 15 mm from the distal end of the guide needles into the perineural spaces. After the catheter is installed to the radial nerve and the catheter to the median nerve and ulnar nerve, the catheter is fixed for the prolonged radial nerve block under the skin of the subclavian region with the help of the first Tuohy guide needle and a spinal needle with a pencil sharpening of the distal end, then the distal end. With such fixation, the inner part of catheters is 140 mm, and the exit point of the catheters is located on the skin of the subclavian region.
EFFECT: method provides the possibility of performing long-term and qualitative analgesia of the operated upper extremity in the area of innervation of all three main nerves of the brachial plexus and enables reliable fixation of the catheter for the prolonged radial nerve block and the catheter for the prolonged median nerve and ulnar nerve block and to carry out due to peculiarities of catheter location under skin of subclavian region and increase of inner part of catheters to 140 mm.
