FIELD: medicine; anesthesiology; intensive care.
SUBSTANCE: invention can be used for prolonged blockade of the brachial plexus. Local anesthesia with lidocaine solution is performed. A skin incision is made and a skin wound is created, followed by expansion with a surgical clamp. The Tuohy needle is passed through the wound to the brachial plexus, then the catheter is passed to the brachial plexus, then the Tuohy needle is removed and this needle is passed through the lateral edge of the skin wound. A spinal needle is passed through the lumen of the Tuohy needle, the Tuohy needle is removed from under the skin of the subclavian region and the proximal end of the catheter is connected to the distal end of the spinal needle by passing the distal end of the spinal needle into the lumen of the catheter at a distance of 3 mm. The spinal needle with the catheter fixed on it is removed from under the skin of the subclavian region until the catheter is completely immersed under the skin of the subclavian region. The catheter is fixed at the site of exit to the skin of the subclavian region with a Perifix fixing device, then the proximal end of the catheter with the catheter connector attached to it and the antibacterial filter are fixed on the skin of the subclavian region on the opposite side of the blocked limb.
EFFECT: method provides reliable fixation of the catheter without the risk of dislocation complications, allows to safely insert the catheter in the subcutaneous tunnel, creates conditions for caring for the catheter which is convenient for personnel, allows to perform ultrasonic visual control over the spread of the local anesthetic solution injected to the brachial plexus due to a set of techniques of the claimed invention.
