FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to oncology, coloproctology. After extralevator rectal extirpation, thin muscles are separated from both lower extremities by cutting off distal portions from common tendon of thigh muscles and mobilization to upper and middle one-third through three separate incisions in places of projection of small muscle. At the next stage, a subcutaneous canal is formed between the mobilized fine muscles and the perineal defect, through which the muscles are brought into the perineal wound and fixed to the walls of the pelvis and the fascia of the greater gluteus muscle. Mesh endoprosthesis is placed on the external surface of the muscles and fixed to the walls of the pelvis and fascia of the gluteus maximus muscle. Through a separate approach to the mesh endoprosthesis, silicone tubes d=0.9 cm are supplied, which are connected to a vacuum drainage. Thin muscles close the perineal defect and delimit the mesh graft from the abdominal cavity.
EFFECT: method makes it possible to perform complete closure of perineal defect after extralevator rectal extirpation, to prevent early postoperative complications, to prevent formation of perineal hernias after performed operation, as well as to improve the patient’s quality of life, since after the operation the patient can be in a sitting position without pain.
