SUBSTANCE: invention refers to medicine, namely to surgery, oncology and proctology. De-epithelised gluteus cutaneous musculocutaneous flap is cut out from one side of the wound by mobilizing 4-5 cm wide flap along the whole length of the wound. A medial portion of the gluteus maximus muscle is transected from a proximal attachment point of the muscle to a posterior and lateral surface of the sacrum and coccyx. Produced flap is immersed into the small pelvic aperture and fixed with single sutures to the residues of the levator ani muscle of the opposite side. Pedicle cutaneous-subcutaneous-fascial triangular flap is cut out from the opposite side of the wound. Wherein, the flap is cut out at angle of 60-80 degrees from the middle of the wound edge, and the sides of the triangular flap should be equal to half the length of the wound edge, is moved, remaining wound cavity is filled with the flap so that the apex of the triangular flap is aligned with the apex of the perineal wound, the triangular flap is fixed with separate interrupted skin sutures.

EFFECT: method provides the qualitative closure of the deep and superficial defect of the perineal wound, the adequacy of the blood supply of the displaced grafts, filling the wound cavity, reducing the number of patient days by 10-15% and the length of rehabilitation by 15-20%.
