METHOD FOR COMBINED PLASTIC SURGERY OF PERINEUM AFTER ABDOMINAL-PERINEAL EXTIRPATION OF RECTUM Russian patent published in 2024 - IPC A61B17/00

SUBSTANCE: invention refers to medicine, namely to abdominal surgery, oncology and coloproctology. Pre-sacral cavity is repaired by a displaced greater omentum. That is ensured by mobilizing a greater omentum along a greater curvature of the stomach from the right edge, ligating a right gastroepiploic artery, preserving a perigastric arch and a left gastroepiploic artery. Left half of the colon and sigmoid colon is mobilized. Then the omentum is brought down to the small pelvis, into the left lateral canal behind the left half of the large intestine, the presacral cavity is filled with an omental transplant. It is fixed along the periphery to the pelvic peritoneum. Then the defect of the perineal wound is closed with a medial portion of gluteus maximus muscle.

EFFECT: method enables to completely close the defect of the perineal wound due to the body’s own tissues, without using synthetic implants; increase supply of plastic material due to two sources of donor tissues; increase total strength of the obtained structure; prevent tension and cutting of sutures of pelvic peritoneum, due to exclusion of suturing on it; reduce the amount of wound discharge; accelerate wound repair processes; reduce risks of formation of local wound complications and bedsores of synthetic implants; reduce probability of perineal hernia formation.
