FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to oncology and plastic surgery. After the subcutaneous radical mastectomy, a lower edge of a greater pectoral muscle and a serratus anterior are separated, thus forming a subpectoral pocket. A semicircle is cut out of the perforated porous membrane (PTFE), wherein its base corresponds to the length of the neo-inframammary fold, and the height corresponds to the length of half the circumference of the silicone implant. A membrane base is anchored in a projection of a neo-inframammary fold to a thoracic fascia with a continuous suture with an absorbable atraumatic vicryl 3/0 suture. A silicone implant is placed in the subpectoral pocket, and the lower pole of the installed silicone implant is covered with a membrane while covering the bottom of the subpectoral pocket. A free semi-circular edge of the membrane is matched with the lower edge of the greater pectoral muscle and the serratus anterior, and a continuous suture is applied with an absorbable atraumatic suture of vicryl 3/0.
EFFECT: method provides the preservation and strengthening of the IMF during the surgical treatment of breast cancer in the given category of patients and, accordingly, obtaining, subsequently, long-lasting satisfactory cosmetic effect with simultaneous significant reduction of risk of complications, by eliminating the possibility of developing surgical aggression with respect to the tissues surrounding the mammary gland as a result of using the bioinert material PTFE, which is used to confine the silicone implant.
