FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to thoracic surgery. After sanitation of the pleural cavity from the lateral approach under the control of a videocoscope, the cutaneous-subcutaneous flaps are prepared from the latissimus dorsi muscle along its entire length with the abduction of the cutaneous-subcutaneous flaps by hooks and tissue preparation. Then latissimus dorsi muscle is cut off from fixation points to ribs, spine and humerus. 5 cm long fragment of 3 rib is resected. Latissimus dorsi flap is delivered into the pleural cavity. In the distal part of the flap of the latissimus dorsi muscle, it is stratified to depth of 6 cm and a portion of flap 3 cm wide and 6 cm long is formed. Formed part of the flap is delivered around the tracheal bifurcation and fixed with separate U-shaped sutures along the edges of the flap to the tracheal wall from the central bronchial stump and the wall of the opposite main bronchus more peripherally to the stump and finally to the adducting part of the flap so that the whole flap is tightly attached to the stump area. Remaining free distal part of flap of the latissimus dorsi muscle is fixed in dome of pleural cavity and to tissues of mediastinum. Pleural cavity and the bed of the mobilized latissimus dorsi muscle are drained separately with tubular drains. Mobilized subcutaneous flaps are fixed in staggered pattern to the underlying tissues with absorbable sutures. Incisional wound is closed in layers.

EFFECT: method enables reducing the incidence of major bronchus stump development, the level of postoperative lethality, reducing the length of postoperative treatment of bronchopleural complications following pneumonectomy in case of a high risk of bronchial fistula and in bronchial stump resection for its inconsistency.
