SUBSTANCE: invention relates to medicine, namely thoracic surgery. After removal of the lung, a diaphragmatic flap is formed on the feeding leg. Next, sutures are applied along the periphery of the stump. In this case, the first suture is applied in the center of the machine stitch. Next, two sutures are applied clockwise on the anterior surface of the trachea: the first is 2 cm above the stump, the second is at the level of the tracheobronchial angle. Next, two sutures are applied to the left main bronchus along the cartilaginous part 2 cm below the stump. Then one suture is applied along the posterior contour of the trachea to the prevertebral fascia. The last suture is applied to the side wall of the trachea 2 cm above the stump above the azygos vein. After that, the azygos vein is mobilized from the lateral wall of the trachea. The distal end of the diaphragmatic flap is brought to the stump of the main bronchus with rotation so that the stump is covered by the abdominal surface of the flap.

EFFECT: method makes it possible to prevent bronchopleural fistula after right-sided pneumonectomy with destructive pulmonary tuberculosis with drug-resistant pathogen, increase the reliability of sealing the bronchial stump, reduce the risk of postoperative complications.
