SUBSTANCE: invention relates to medicine, namely thoracic surgery. Mobilization of the flap of the broadest back muscle is carried out. A section of the rib is resected along the mid-axillary line to conduct a flap. 3-4 single ligatures are applied through the stump of the bronchus without tying them. The muscle flap is folded in half along its longitudinal axis. The upper end of the thread is passed through the overlying part of the muscle, and the lower end is passed through the underlying one with a puncture back through the muscle flap without capturing the bronchus. The nodes are tightened, avoiding flap ischemia. After applying 3-4 stitches through the stump of the bronchus, additional fixation of the flap is performed to the side wall of the trachea, the main bronchus, the lobar bronchus, the prevertebral fascia, the mediastinal pleura.

EFFECT: method makes it possible to eliminate bronchial fistula, residual pleural cavity, prevent recurrence of bronchopleural communication and the development of pleural empyema.
