FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to vascular surgery, and can be used in surgical treatment of abdominal aortic aneurysm (AAA), mainly of its infrarenal part. After opening the aneurysmal sac, a working portion of a retractor is inserted into its cavity with the number of teeth not less than 3, having a length of the working portion determined by the distance between the extreme teeth not exceeding the length of the aneurysm. Wound retractor is positioned longitudinally with its teeth located in the distal direction from the neck of the aneurysm with further separation of its branches. When forming a proximal anastomosis between the prosthesis and the aorta, a U-shaped suture is applied in the upper right corner of the aorta, which corresponds to 12 o'clock of the conventional clock face. Suture is tied, then an anastomosis is formed along the anterior wall from 12 o’clock to 3 o’clock with a continuous blanket suture. Prosthesis is retracted in the wound laterally, an anastomosis is formed along the posterior wall, then the anastomosis is formed on the remaining part of the anterior wall.

EFFECT: method simplifies the surgical management of AAA, provides an adequate view when forming a proximal anastomosis between the synthetic prosthesis and the aorta, reduces intraoperative complications by using a retractor inside the aneurysmal sac.
