FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to traumatology and orthopaedics, and can be used in performing locked intramedullary osteosynthesis of long bones. Rod is implanted and the rod is blocked distally and proximally with screws. After implantation of the locking rod, a closed decompression fasciotomy of the posterior superficial and posterior deep osteofascial cases is performed simultaneously with the help of a fasciotome. Fasciotome comprises two cutting heads with a fixed distance between them of 15 mm. The cutting heads of the fascilotome from the skin incision are placed on the fascia; the first cutting head is placed under visual control, and the posterior surface case is decompressed. Second cutting head is used to decompress the rear deep case parallel to the first cutting head. When the fasciotome is advanced in the distal direction to the lower one-third of the shin, a decompression fasciotomy of both said cases is performed. Fasciotomy of anterolateral and lateral cases is performed simultaneously by means of fasciotome from incision for implantation of intramedullary fixator located in upper one-third of shin. First cutting head is placed under visual control on the fascia forming the anterolateral case, the second one – above the fascia forming the lateral case. When the fasciotome is advanced in the distal direction to the lower one-third of the shin, decompression fasciotomy of both said cases is performed and sutures are applied.
EFFECT: method provides prevention of local hypertensive ischemic syndrome, does not require additional tissue injuries, excludes deviations in the technique of fasciotomy of these shin cases due to simultaneous decompression.
