FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to neurosurgery, surgery, urology and coloproctology. Operation is performed under combined endotracheal anaesthesia. Urethral catheter is inserted into the bladder under sterile conditions. Trocar incision is made in the paraumbilical region – under the navel, 1 cm long. A main trocar is inserted through the incision into the abdominal cavity, with the help of which a pneumoperitoneum of up to 12-14 mm Hg CO2 is applied. A laparoscope is then inserted through said trocar. Further, two working 5 mm trocars are installed on the left and on right 3 cm below the navel along the mid-clavicular line. Laparoscopy is performed, during which the abdominal cavity is examined. Abdominal, abdominal and pelvic abnormalities are examined. Further, the following stages i1, i2, i3, i4 are performed on the side of pudendal nerve compression: i1 – peritoneum is opened, i2 – retroperitoneal space is dissected in the region of the obturator opening and the ischiorectal space, the sacrospinous ligament is visualized and reached, i3 – using transvaginal palpation under control of laparoscope visualizing ischial tuberosity and sacrospinous ligament, i4 – a sacrospinous ligament is dissected along its entire length medially and from above the ischial spine with scissors or an ultrasonic scalpel, and haemostasis is performed. Instruments and trocars are removed from the abdominal cavity; the pneumoperitoneum is removed. Trocar holes are closed with separate sutures, aseptic dressings are applied.

EFFECT: method is less traumatic, enables to preserve a large array of tissues surrounding the genital nerve, which significantly reduces the amount of blood loss, the volume and time of wound surface healing, reduces the pain and drug load on the patient's body, reduces the length of staying in hospital, postoperative rehabilitation, recovery of lost functions.
