METHOD OF SURGICAL APPROACH TO GENITAL NERVES FOR THEIR DECOMPRESSION Russian patent published in 2024 - IPC A61B17/00

SUBSTANCE: invention refers to medicine, namely to surgery and neurosurgery. Transgluteal skin incision 8 to 10 cm long is performed. Skin incision starts from a sacrococcygeal joint at 2–3 cm from a median line and continues in an arc-like manner downwards to a projection of the ischial tuberosity. Then soft tissues are moved aside, gluteus maximus muscle is pulled apart in blunt way, internal fascia of gluteus maximus muscle is dissected, and sacral-tuberous ligament is visualized at its attachment point to ischial tuberosity. At this stage, the anterior surface of the ligament is probed, and the genital nerve is visualized under the ligament. Distal point of entry of nerve into Alcock’s canal is determined. Grooved probe is also introduced into the canal, through which its medial wall is dissected, the genital nerve is separated in the interligamentous space and at the level of the Alcock’s canal, and its decompression is performed along the whole length.

EFFECT: method enables reducing injuries and labour intensity of the surgical intervention by performing a modified skin incision and preserving the integrity of the muscle.
