FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to orthopaedics, and can be used for surgical treatment of pronation contracture in congenital proximal radionuclide synostosis of forearm bones. Method includes transverse destruction of integrity of ulnar bone, intramedullary wire conduction, osteotomy of radial bone, segmental resection of bone fragment, derotation manoeuvre to medium physiological position of forearm with transverse fixation of position with pins. Pin is delivered through an ulna intramedullary. Method includes osteoperforation of middle one-third of ulnar bone, held percutaneously and transversely, and closed osteoclasia. Zone of synostosis is separated by a longitudinal incision along an external-lateral surface of a forearm in a projection of radio-ulnar synostosis. Subperiosteal oblique osteotomy of the radial bone is performed in a horizontal plane along a line of fusion with an ulnar bone; a second oblique osteotomy of a proximal end of the radial bone is performed in a sagittal plane longitudinally, along a line from a lower external point of a synostosis of the radial bone to an upper internal point of the radial bone. At an angle to the plane of the performed radial osteotomy, which is equal to the angle of the pronation positional installation of the forearm, a bone wedge is excised from the proximal end of the distal fragment of the radial bone. Forearm is derotation to be placed in a mid-physiological position, transversely with K-wires, and a radius bone is fixed to an ulnar bone. Wound is closed tightly in layers.
EFFECT: method provides reduction of surgical injuries during correction of pronation to functional position of forearm without tension of soft tissues, stability of achieved positional position of forearm and prevention of recurrence of pronation deformation in remote period, as well as reduction of osteotomy fusion time with improvement of anatomical outcome with less pronounced cosmetic defect due to intramedullary guide wire, performing osteoperforation of middle one-third of ulnar bone percutaneously and transversely and subperiosteal conducting oblique osteotomy of radial bone.
