FIELD: medical science.

SUBSTANCE: invention relates to cardiovascular surgery, specifically to a method for surgical treatment of thoracoabdominal aortic aneurysm (TAAA), as well as aortic dissection (AD), and can be used in specialized departments of cardiovascular surgery to prevent development of pain syndrome in postoperative period. For this purpose, after the thoracoabdominal aorta reconstruction at the end of the operation, a cryoprobe is placed in the thoracotomy region in a certain manner. Cryoablation is performed. Cryoprobe is displaced into the other intercostal spaces, and cryoablation is performed once more. For each exposure cryoablation is performed for 30-60 seconds at temperature -140 °C under the control of electroneuromyography (ENMG), while stimulating with current intensity starting from 5 mA to gradually increased to 10 mA to block the intercostal nerves according to ENMG results.

EFFECT: method provides preventing the onset of pain syndrome, which can substantially occur in the postoperative period after the reconstruction of the thoracoabdominal aorta, due to the intercostal nerves cryoablation performed by the developed method, which allows to perform single-step action on all intercostal nerves in the area of interest, which will allow to prevent pain syndrome more quickly, conveniently and sparingly for the patient due to reduced time of both surgical intervention and duration of anaesthesia, which accordingly affects the economic component, in comparison with the known selective cryoablation of intercostal nerves.
