METHOD FOR PRACTISING CLOSED ACHILLOTOMY Russian patent published in 1997 - IPC

FIELD: medicine; methods for surgical treatment of locomotor system diseases, in particular, patients affected with neuromuscular diseases, such as talipes equinus. SUBSTANCE: achillotome is introduced into middle of distal end of Achilles tendon so that achillotome tip would pass under tendon to one half of its length. Next tendon is pierced from its front backwards and, without damaging skin (for this purpose mark is left above achillotome tip), tendon is dissected longitudinally from top downwards in sagittal plane, while one half of its portion is dissected transversely in frontal plane. Second point, at which achillotome is inserted by piercing, is located somewhat below muscular belly, at tendon's middle. Instrument is made to pass under tendon and its tip is drawn to emerge just at mark point. Without damaging skin, surgeon must dissect tendon longitudinally from below upwards to thereby combine two incisions into single one in sagittal plane. Same procedure is practised in opposite half of tendon transversely in frontal plane. Respirator is introduced through opening pierced in skin at calcanean tuber, followed by subcutaneously dissecting cicatrices formed between skin and tendon. Foot is drawn at right angle to shin. EFFECT: reduced rate of traumatism caused by operation; reduced rate of postoperative complications; operation takes less time; reduced number of surgeons engaged in operation; reduced consumption of surgical material. 1 cl
