FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to orthopaedics, and can be used for treating equinus deformity of foot in patients with infantile cerebral paralysis and clubfoot. In the projection of the Achilles tendon entensis and in the point of transition of the Achilles tendon into the muscular part, 2 skin incisions are made on both sides of the tendon, each being up to 10 mm long. A surgical suture is inserted through one of the proximal incisions between the gastrocnemius and soleus muscles, the ends of which are brought into the peritendon on both sides of the Achilles tendon and brought out through the distal skin incisions in the enthesis projection. Achilles tendon is sawed in a frontal plane into a gastrocnemius portion and a soleus portion. Through one of the skin incisions in a projection of the distal portion of the Achilles tendon between the gastrocnemius portion and a soleus portion, the achillotome is inserted and delivered to the middle of the tendon. Gastrocnemius portion of the Achilles tendon is pierced with an achillotome cutting point from the front to the back without damaging the skin. Label is made on the skin above the palpable end of the achillotome. Achillotome is used to dissect a distal portion of a gastrocnemius portion of the Achilles tendon from top to bottom in an oblique direction to an internal point of the Achilles tendon entensis. Achillotome are repeatedly introduced through one of the skin incisions in a projection of the proximal portion of the Achilles tendon between the gastrocnemius and soleus portions of the tendon with its point to label. Gastrocnemius portion of the Achilles tendon is punctured with an achillotome point from the front to the back, and a proximal portion of the gastrocnemius portion is incised in an oblique direction from the bottom up, connecting two incisions into one. Foot is taken out of the vicious position. Plaster boot is applied from the toes to the upper one-third of thigh for 6 weeks.
EFFECT: method provides preservation of soleus muscle function and excludes development of recurrent deformation in remote postoperative period due to peculiarities of Achilles tendon repair.
