FIELD: construction engineering; erection of monolithic reinforced concrete structures. SUBSTANCE: form has modular symmetrical frame shields made of plywood deck placed in frame made of contour profile and strengthened by transversal sections. Contour and transversal sections are made of extruded high-strength aluminum alloy and have in their sections rectangles whose smaller sides are reinforced. Contour section has at least one inner crosspiece joining opposing longer sides of rectangle; U-shaped slot is made in one longer side of rectangle. Flanged lip is made at point where this side is joined to plywood deck and reinforced side of rectangle to form depression in the latter. In addition, contour and transversal sections are provided with reinforcing members. Thickness of section walls is varying throughout cross-sectional area. Transversal section is provided with outer longitudinal slot made on smaller reinforced wall; head of self-locking bolt of strut and scaffold fastening is free to move over this slot. Outer surface of larger walls is provided with notches. EFFECT: reduced material consumption at same stability and service life. 4 cl, 4 dwg
