FIELD: medicine, neurosurgery. SUBSTANCE: in case of cerebral edema in patients with a severe cranial trauma at the development of hyperosmotic syndrome one should preliminarily conduct hemodiafiltration at the rate of controlled hemofiltration being 400 ml/h for 4-5 h. Removed volume of 1200-1600 ml hemofiltrate is compensated with a Ringer's solution at the same volume up to normalization of plasma osmolarity followed by mannitol solution infusion at the dosage of 1 g/kg under the control of intracranial pressure. EFFECT: higher efficiency of therapy.
