FIELD: medicine, traumatology. SUBSTANCE: one should apply vertebral distractor which has got a distracting part in the form of flats located at the angle of 90 deg. at cylindrical rod, in sleeve-guide a cylindrical part has a whisker to be introduced into interarch space and lateral access located laterally towards articular processes. Distractor should be introduced into interarch space and rotated for the angle of 90 deg., then sleeve-guide is introduced with its whisker at medial direction along spinous process, a cutting is introduced along sleeve-guide to form a channel in interbody and interarch spaces by a fixator that prevents damage of vascular-nervous plexus and facets of processes. Distractor consists of cylindrical rod, caput and working part with two planes. Rod has got a distracting part as two flats. Sleeve-guide consists of caput and cylindrical part with triangular tenons. Cylindrical part has got whisker to be introduced into interarch space and P-shaped lateral access. The latter is less than one third of cylindrical part diameter located laterally towards articular processes. The innovation enables to decrease the number of invalid patients, improved patients' life quality in case severe degenerative lesions after operative interferences.
