FIELD: production of data medium used for computer identification of records. SUBSTANCE: formatted data medium, primarily printed form for computer-aided input in data system, has data fields disposed in subject sequence and provided with permanent subject font data headings and at least one window for entering character of individual significant information isolated by explicit separators affording location of window for its filling. Separators on at least two sides of window are made in the form of gaps filled with white-and-black raster of printed characters spaced apart through at least two-fold maximal linear size of signs chosen proceeding from condition that sign appearance together with character entered in this window is excludes and that it does not exceed three-fold linear size of scanner decomposition element designed to read data out of printed form.
