FIELD: medicine, thoracic surgery. SUBSTANCE: one should carry out cross-sectional cutaneous incision. Cutaneous fragments are exfoliated with subcutaneous-fatty fiber to open sternocostal complex. Xiphoid process should be dissected together with rectus muscle of abdomen from sternum. Bilateral resection of cartilaginous areas is performed within IV-VI ribs area. Cross-sectional sternotomy is conducted followed by dissection of VI, VII, VIII ribs from sternum to be tightened in medial direction by applying them, moreover, onto sternum. At achieving maximum possible costal shift, their surface should be marked with labels at the level of anatomic fixation to sternum to carry out resection. After that shortened ribs should be sutured towards sternum. Then xiphoid process and rectus muscle of abdomen are fixed to sternum. The present method enables to provide reliable fixation of sternocostal complex by keeping its anatomic nature. EFFECT: higher efficiency of operation. 2 dwg
