SUBSTANCE: one should perform fibular osteotomy being above articular fissura, dissect anterior ligaments of tibiofibular syndesmosis and that in top area of external ankle, rotate it outwards, perform vertical osteotomy of internal ankle together with tibial distal metaepiphysis and dislocate it outwards, remove cartilages from articular surfaces of tibia, fibula and ankle bone, perform segmentary osteotomy in internal department of tibial distal metaepiphysis to further form bony porous autotransplant being equal to defect's height of talocrural articular fissura and introduced into fissura developed in talocrural joint, both internal and external ankles should be fixed to tibial epiphysis with porous screws. At achieving arthrodesis it decreases articular volume that enables to wear cosmetically matched footwear.
EFFECT: higher efficiency of operation conducted.
