SUBSTANCE: one should perform an incision along the edge of rectangular cartilage, due to layer-by-layer mucosal and perichondrial dissection open caudal edge of nasal septum, separate mucoperichondrial fragment at concave side, then comes separation of nasal septum foundation acutely by opening premaxillary area. Then it is important to dissect the cartilage against perpendicular plate of ethmoid bone to remove cartilaginous strip of 1 mm along superior edge of nasal septum along with introducing caudal edge into incision between median stems of alar cartilages. Cartilage should be fixed with interrupted sutures with surgical sutures being a ligature based upon titanium nickelide of TN-20 type, 0.12 mm diameter by making two subcutaneous sutures of caudal edge. Additionally, one should form two cutting foramens of 0.3 mm diameter in osseous premaxilla to fix posterior cartilaginous edges with two sutures through these cutting foramens. The method enables to provide reliable fixation of nasal septum cartilage in correct position, prevent repeated deformation, restore nasal respiration and, also, remove cosmetic defect successfully.
