FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to otorhinolaryngology, and can be used in reconstructive-plastic surgery of craniofacial area and for rhinoplasty. Anaesthetic solution is injected and the nasal mucosa is separated along the nasal septum and the lateral wall on the side of the cleft. Then a skin portion of the nasal septum is incised on the side of the cleft. It is followed with complete isolation of quadrangular cartilage along its entire length and its resection. Two columns are formed from the remains of quadrangular cartilage after restoration. One of the columns is implanted into the caudal portion of the nasal septum between the medial pedicles of the wing cartilages. Second column is used to form a nasal valve angle. Deformed premaxilla is resected, from the removed portion of which a bone autograft is made to form a nasal vestibule on the side of the cleft. Second, linear incision on the side of the cleft is performed between the wing cartilages along the edge of the pear-shaped opening. To form a lateral wall of the nose valve, a bed is formed below and medial to the lateral pedicle of the cartilage, where the second column of the autologous cartilage is placed. Then, on the side of the cleft on the vestibule bottom, a bed is formed by separation of tissues, where a pre-prepared bone autograft from premaxillum is placed so that edges of the bone autograft are in contact with the edges of the implanted autoglossial columns. Formed bottom of vestibule of nose is covered with soft tissues, mucoperichondria and skin are laid back and sutured with absorbable sutures. Elastic tampons are placed in the nasal passages, a splint is applied on the dorsum of nose and the tar-like dressing. Tampons are removed on 4th postoperative day.

EFFECT: method enables to obtain a good functional and aesthetic effect, restore the nose valve, vestibule of the nose, intranasal structures due to endonasal approach, use of bone and cartilaginous autografts.
