METHOD FOR DRAINING POSTERIOR PURULENT MEDIASTINITIS Russian patent published in 2006 - IPC A61B17/00 

FIELD: medicine, surgery.

SUBSTANCE: the present innovation deals with treating purulent mediastinitis with the help of extra-pleural access to posterior mediastinum in inferior department. For this purpose, a patient should be put into stomach-position to perform a linear incision above the 11^th rib being left-hand against vertebral column beginning from external edge of extensor muscle of back of about 9-11 cm length along with dissecting the broadest muscle of back, posterior inferior toothed muscle and intercostals muscles, as well. Then one should medially remove extensor muscle of back to perform resection of the 11^th rib. After this procedure, it is necessary to exfoliate pleura against vertebral column to penetrate posterior inferior medistinum. The innovation enables to provide vast penetration into posterior inferior mediastinum to have the chance for performing manipulations at inferior-thoracic esophageal department and exclude pus congestion in inferior section of posterior mediastinum due to applying a drainage tube in the most inferior site of mediastinum.

EFFECT: higher efficiency of drainage.
