FIELD: medicine, thoracic surgery. SUBSTANCE: one should perform resection of anterior arch of 4th rib right-hand. Pleural cavity is lanced through its bottom. Operative gauze tampon on curved clamp is directed along diaphragmatic-mediastinal pleural sinus towards posterior cardiac border. This clamp is used to destroy mediastinal pleural folium at posterior-inferior cardiac contour. Soft tissues from right hypochondrium are punctured into anterior costal-diaphragmatic pleural sinus. One should direct the end of drainage tube with lateral openings into posterior costal-diaphragmatic sinus through anterior costal- diaphragmatic pleural sinus. Drainage is fixed to skin. Edges of operational wound are sutured layer-by-layer. The present method enables to decrease the number of complications at drainage access to posterior mediastinum.
