SUBSTANCE: one should remove mediastinal lymph nodes due to endoscopic technique. Moreover, it is necessary to fulfill abdominal access due to superior-median laparotomy and sagittal diaphragmotomy. Access should be carried out without any interference upon right-hand pleural cavity and lancing mediastinal pleura. The innovation enables to decrease operational trauma and shorten time of operation.
