SUBSTANCE: in patients with Chiari's malformation type I in osseous defect in the site of great occipital cisterna it is necessary to fulfill a P-shaped incision of external leaf of dura mater, its bottom being downwards, exfoliate it against internal leaf of dura mater to incline it downwards to posterior atlas' semiarch. Then one should lance internal leaf of dura mater due to P-shaped incision with its bottom at the top to incline it upwards towards the edge of great occipital foramen. Then it is important to carry out tonsilar photocoagulation with infrared laser and suture the edges of the developed fragments of dura mater due to "sail"-type technique. The innovation is successful due to creating valuable great occipital cisterna and permeability of subarachnoidal space.
EFFECT: higher efficiency of plasty.
