FIELD: medicine, neurosurgery.

SUBSTANCE: the present innovation deals with protecting the brain against ischemia in case of surgical interferences in area of internal carotid artery and bifurcation of common carotid artery. Thus, 12 h before operation one should intravenously inject perfluoran at the dosage of 5 ml/kg body weight and simultaneously it is necessary to inhale oxygen-enriched air mixture. Just before interference it is necessary to squeeze arterial flow being below and above the site of manipulations followed by longitudinal dissection of common carotid artery by coming onto internal carotid artery. Then through arteriotomal foramen one should cannulate the mouth of external carotid artery, through which one should additionally introduce perfluoran at the dosage of 5 ml/kg. On finishing the manipulations upon an artery one should suture in arteriotomal wound, moreover, before applying the last sutures one should remove the cannula. Then it is important to reconstruct circulation; during 12 h after operation a patient should again inhale oxygen-enriched air mixture. The innovation enables to increase the range of manipulations upon carotid arteries and their duration without any considerable disorders of cerebral circulation and, also, improve cerebral oxygen supply during operation due to applying perfluoran.
