SUBSTANCE: simultaneous isolation of the common carotid artery (OCA), external carotid artery (ECA), internal carotid artery (ICA) and false aneurysm is performed. The great saphenous vein (GSV) is isolated by 7 cm and removed from the wound to the selected length. The semilunar valves are removed. ECA, CCA, ICA are clamped with vascular clamps for 30 minutes and a false aneurysm located in the area of the common, internal and external carotid arteries is removed. Then, end-to-end anastomoses are performed between the proximal end of the GSV and the ECA, then between the distal end of the GSV and the ICA. An arteriotomy hole 1 cm in diameter is cut in the middle of the GSV in the wall adjacent to the lumen of the CCA. After that, the vascular clamp is removed from the ECA for 3 seconds, followed by the removal of the vascular clamp from the ICA also for 3 seconds. Next, an end-to-side anastomosis is performed between the CCA and the arteriotomy opening in the GSV, after which the vascular clamps are removed from the ECA and then from the CCA, then the vascular clamp is removed from the ICA.

EFFECT: method reduces the risk of intraoperative ischemic stroke and ischemic stroke under conditions of occlusion or restenosis of the ipsilateral ICA in the late postoperative period by reducing the time of arterial clamping and maintaining blood flow through the ECA.
