METHOD FOR PLASTY OF THE DEFECT OF TRACHEAL ANTERIOR WALL Russian patent published in 2007 - IPC A61B17/24

SUBSTANCE: the present innovation deals with removing the tracheal defect with the help of a transplant pre-obtained out of a cartilage to be applied onto tracheal defect. Moreover, the latter should be removed with a combined fragment out of arterialized muscular-fascial fragments of sternocleidomastoid muscles with a costal allocartilage and a fascial stopper inside. One of the two muscular-fascial fragments should be applied through the defect to be fixed with sutures towards the opposite deepithelized edge of laryngeal defect, and then one should apply a strip of allocartilage onto this layer in transverse direction. Moreover, it is necessary to specify laryngeal and tracheal cartilages and fix allocartilage to their perichondrium with allotendinous ligatures, and as for the juncture between tracheal cartilage and allocartilage - it should be restricted with allogeneic fascia. Then it is necessary to apply the second muscular fragment above and suture it to the opposite deepithelized edge of laryngotracheostoma, and suturing in of external wound should be fulfilled on finishing the mobilization of skin edges. The innovation enables to reconstruct step-by-step the anatomical laryngeal and tracheal integrity at forming a rigid wall and, also, prevent the lysis of allocartilaginous tissue due to applying a fascial stopper.
