FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely to surgery and otorhinolaryngology. Before the operation, the required number of grafts from sterile rib allocharl is modeled in accordance with the dimensions of the three-dimensional prototype model. The prototype model recreates in full size the normal anatomical parameters of the tracheal half ring and has the shape of a half ring cut into three parts with a diameter of 30 mm. The length of the projection of the arch of the lateral graft is 25 mm, the length of the projection of the arch of the anterior graft is 20 mm, the height of the graft is 5 mm, and the width is 3 mm. The resulting grafts are perforated with through holes. Then, at the first reconstructive plastic surgery after excision of the scar tissue, a new lumen of the larynx and trachea is formed. In the stenosis zone, muscular-fascial pockets are formed paratracheally, into which the required number of lateral grafts is placed. They are fixed to the surrounding tissues with Vicryl 3.0, covered in layers with muscles, fascia, subcutaneous fat, and the skin is fixed to the tracheal wall. Carry out prosthetics on a T-shaped tracheostomy tube from 3 to 6 months. Then, a control period is carried out without a tracheostomy tube with a hermetically sealed laryngotracheostomy opening. At the second reconstructive plastic surgery, the required number of anterior grafts are implanted in order to form the anterior wall of the larynx and trachea. The anterior grafts are fixed to the previously implanted lateral grafts. The laryngotracheostomy defect is sutured and the laryngotracheostomy is closed.
EFFECT: method allows to increase the efficiency of treatment, reduce the number of stages of reconstructive operations in the treatment of patients with extended laryngeal tracheal obliteration, reduce the likelihood of postoperative complications, prevent restenosis of the larynx tracheal lumen, eliminate the need for repeated surgical interventions, reduce the duration of inpatient treatment and terms of rehabilitation of patients.
