METHOD FOR DEVELOPING A "DRY" ACCUMULATING UROSTOMA Russian patent published in 2008 - IPC A61B17/00

Abstract RU 2320279 C1

FIELD: medicine, urology.

SUBSTANCE: the present innovation deals with treating patients at the deficiency in ureters' length who passed cystectomy due to different indications. As a single unit one should mobilize the part of terminal ileal department, caecum and ascending part of colon up to hepatic curvature to create a low-pressure reservoir to accumulate urine at volume being 400 ml. A "short" right-hand ureter should be anastomosed with the wall of the developed accumulating reservoir. A "short" left-hand ureter should be sutured by "end-to-end" type at terminal department of ileum being 40 cm. For preventing incontinence of urine it is necessary to isolate the part of small intestine of 15 cm length being narrowed up to the diameter of a catheter N12 according to Charrier scale. The part of small intestine should be arranged antiperistaltically to then sutured with proximal end towards the reservoir's wall. Moreover, one should invaginate the mucosa in the site of anastomosis for creating additional resistance to urinary outflow. With distal end the part of small intestine should be sutured towards abdominal wall by creating a "dry" urostoma. The innovation enables to increase patients' standard of life due to providing urinary outflow through a "dry" urostoma at short length of ureters that restricts the chances to carry out operation on reconstructing urinary outflow with functional techniques.

EFFECT: higher efficiency.
