FIELD: medicine.

SUBSTANCE: prostate-preserving cystectomy is done with retention of apical part of the prostate gland and its neurovascular bundles. The terminal portion of the ileum with length 15 cm is resected together with the blind and part the ascending colon on a feeding vascular pedicle. The separated segment is rotated 180 degrees in the frontal plane. The end of the ascending colon of the vessel is isoperistaltically anastomosed with a prostatic capsule by two-row suture. The inner suture is continuous, and the outer suture is interrupted. An antiperiflux mechanism for renal ducts is made, implanting renal ducts in the terminal portion of the ileum above ileocecal valve. The formed vessel is connected to the urethra. The method is technically simple, adjusted to physiology, prevents suture failure due to creation of a vessel with a shape closer to a sphere without detubulisation; allows for fast normalisation of urodynamic performance due to isoperistaltic connection of the vessel to the prostate gland; prevents extravasation of urine and phlegmon due to use of double layer anastomosis.

EFFECT: method minimises the risk of development of metabolic disorders due to resection of a small segment of the terminal portion of ileum, prevents retrograde reflux and ascending infection, prevents uroclepsia in the postoperative period and retention of potency due to preservation of outer transversely striated urethral sphincter.
